Start with routing facts—not a case file.

The initial request needs a town, broad care path, timing and a contact person. It should not contain diagnoses, medical records, medication lists, Social Security numbers, insurance documents or financial-account data.

• Obtain appropriate permission before submitting another person’s contact information
• Use your own contact details if the family has asked you to coordinate
• Select “A friend or client” on the form
• Let any later provider use its appropriate secure intake process
• Call 911 for an emergency; do not rely on the form for urgent response
