All 138 residential care homes, assisted living residences and nursing homes that Vermont licenses, taken from the state’s own published record on August 19, 2026 and organised by county: 87 residential care homes, 18 assisted living residences and 35 nursing homes, of which 29 carry a designated memory care programme and 83 take a Vermont Medicaid residential payment. This is a licence record, not a ranking. Being listed means Vermont licenses the setting; it says nothing about quality, staffing, current openings, price or whether a place suits one particular person. There are no scores here and no reviews, because we have not inspected any of these buildings. Licence status changes. Confirm anything time-sensitive with the provider and with the state before deciding.

What the licence terms on each facility page mean
Residential care home
A residential care home is licensed by the Vermont Division of Licensing and Protection to provide housing, meals, personal care and general supervision. Level III homes provide personal care; Level IV homes provide general supervision without routine personal care.
Round-the-clock skilled nursing is not part of the licence: that belongs to a nursing home. Homes vary widely in size, from a handful of residents in a converted house to purpose-built buildings.
What to verify before you decide:
- Which licence level the home holds, and what that permits staff to do.
- The written service plan, and which tasks are included in the base rate.
- Overnight staffing: how many people are awake and on site.
- What happens if needs increase beyond the licence category.
- The full fee schedule, including care-level surcharges and one-time fees.
- Current licence status, through the Division of Licensing and Protection.
- That the legal entity on the agreement matches the organisation that will deliver the care.
Assisted living residence
An assisted living residence is a distinct Vermont licence category that combines a private apartment with a lockable door and private bath, meals, personal care and nursing oversight, under a model built around resident autonomy and ageing in place.
A nursing home is a separate category entirely. Vermont licenses relatively few of these compared with residential care homes, so choice is limited in much of the state.
What to verify before you decide:
- Whether the apartment under discussion matches the one shown on a tour.
- How care levels are assessed and re-assessed, and what triggers a price change.
- Which clinical tasks the residence performs and which need an outside provider.
- The written criteria for transfer or discharge.
- Deposits, community fees, refund terms and notice periods.
- Current licence status, through the Division of Licensing and Protection.
- That the legal entity on the agreement matches the organisation that will deliver the care.
Nursing home
A nursing home is licensed to provide skilled nursing care and round-the-clock supervision, for people whose needs cannot be met in a residential setting.
Admission normally follows a clinical assessment. Many Vermont nursing homes also provide short-term rehabilitation after a hospital stay, which is a different service from long-term residence.
What to verify before you decide:
- Whether the discussion is about short-term rehabilitation or long-term residence.
- Medicare and Medicaid certification status for the specific bed.
- Current staffing levels and how they are covered overnight and at weekends.
- The most recent state inspection findings.
- How a Medicare rehabilitation stay converts to private pay or Medicaid.
- Current licence status, through the Division of Licensing and Protection.
- That the legal entity on the agreement matches the organisation that will deliver the care.
Assistive Community Care Services (ACCS)
Assistive Community Care Services is a Vermont Medicaid payment that covers personal care, supervision, nursing oversight and activities in a licensed residential setting. Room and board stay outside it, so a resident still pays that housing portion from income or other resources.
Enhanced Residential Care (ERC)
Enhanced Residential Care is a Vermont Medicaid payment for people who need a higher level of service in a residential care home or assisted living residence than Assistive Community Care Services covers. Eligibility runs through Choices for Care and is decided by the state, not by the facility or by this website.
Participation in the state record does not mean a Medicaid-funded place is open now, and it does not mean a particular person qualifies. Eligibility is decided through Vermont’s Choices for Care programme.
Designated dementia or memory care
A state-designated dementia or memory care programme means the setting has been recognised for that specific programme. Quality, staffing ratio and outcome are not what it measures, and it does not mean every part of the building is secured.

Every Vermont county has an Area Agency on Aging that explains Choices for Care and the other public programmes, independently of any one provider. Boundary towns can differ, so confirm the town when you call.
